    And so, she decided she should leave AMA and ended up right back at the hospital in a worse situation than she was when she left. But that's the part that observable piece at the top that people can't identify, even nurses sometimes can't identify what they've done and what kind of clinical reasoning or judgment they've applied. So, Layer 1 is that observable layer that shows you the clinical judgment piece in its one big process there. So, there's no way to measure that and it's really difficult to teach. The second, Layer 2 and 3, these are not observable. They're supportive of Layer 1 and, of course, Layer 0, but Layer 1, 2, and 3 are considered the cognitive layers of this model.

This adaptive process will go on as the exam continues. Please note that the line of item number 7 is much shorter than the line of item number 1. When the computer only receive one response, the confidence interval range is wide, which means the estimation is not precise. When the computer collects several responses, it knows more about the candidate's performance and has 95% confidence that the true ability is within the narrower range.
