So, one thing I just want to note here, and I didn't know this, they really are, to their credit, the Institute for Justice, and Paul Sherman, who's a nice enough guy, is honest and upfront about his intentions to do exactly this, to eliminate licensure. He's telling you to his face, he doesn't want licensure for physicians. He doesn't want licensure for dieticians.

These are also...this wasn't in any way an experimental design. It was basically an association trying to look at variables that could predict various substance usages. So I got some conclusions that I'd like to share with you now. Nurses, then, are at moderate risk for tobacco use, alcohol use, at 11.6%. If you think of 100 nurses, 11, almost 12 of them, are at moderate risk for these substances, and for other substances, about 10%. These are concerning estimates in terms of risk. Trends in substance use in nurses, I believe, warrants further study, both on an individual and system level.

Patricia Benner was writing about this in 2010 in Educating Nurses. That was 10 years ago. We were describing this in the early 2000s as the Institute of Medicine core competencies was moving into the world of QSEN. We were describing the need for this. ©2020 National Council of State Boards of Nursing, Inc. All rights reserved. 4 So, this really isn't new and revolutionary.

Personally, this is set to 10%. So, out of 65 items, we'd expect about six or seven items to be clinical judgment standalone items. If random selection picks for a clinical judgment item, then an optimal standalone clinical judgment item will be selected for the individual. However, if it does not, then we'll kick into the second phase, which will kick in the item selection, which will be based on the content area percentages.

So what are our results telling us? Our results are telling us that nurse practitioner report significantly better work environment in 2018 when we control for individual and organizational characteristics, and positive changes were observed both for experienced and less experienced NPs. One thing we need to understand that, you know, our study took advantage of this natural experiment that happened in New York State to show how the work environment improved for nurse practitioners.
