Delaware Board of Nursing • Inmate at Delaware Department of Corrections facility has Hepatitis C and is prescribed Sovaldi as a treatment ØMedicine strictly monitored by counting pills at every shift change • On 3/15/2015, during a shift change a nurse spilled 12 pills on the floor. The nurse “wasted” the pills into a “sharps” container • Medical Director for firm contracted to provide inmate health care requested Francis to retrieve the pills from the container • Francis and another nurse located the container and retrieved the 12 pills Francis v. Delaware Board of Nursing • After inspection by Francis, another nurse, and the onsite pharmacist, the pills were returned to their original bottle.

When talking about physical and psychosocial consequence, according to the World Health Organization, substance use, namely, alcohol use, have been linked to more than 200 physical and psychosocial illness, including cardiovascular disease, liver cirrhosis, and some types of cancers as well as injuries resulting from violence and road clashes and collisions. In addition, 20% of primary care medical visits, 40% of hospital medical admissions, and 70% of emergency medical visits are associated with substance use. In term of mortality and morbidity, every day, more than 128 American die of an opioid overdose. An estimated 1.7 million Americans suffered from substance use disorders related to prescription opioid.

A case study contains six items that are associated with the same client presentation, share unfolding client information and address the following steps in clinical judgment. Recognize cues – identify relevant and important information from different sources (e.g., medical history, vital signs). Analyze cues – organize and connect the recognized cues to the client’s clinical presentation. Prioritize hypotheses – evaluate and prioritize hypotheses (urgency, likelihood, risk, difficulty, time constraints, etc.). Generate solutions – identify expected outcomes and use hypotheses to define a set of interventions for the expected outcomes.
