Physicians are asking nurse practitioners to practice outside their lane. NPs are particularly vulnerable to this misalignment where they think that having skill sets that they develop on the job, "Oh, well, the supervising physician or the collaborating physician, or my colleague can teach me how to do X, Y, Z." ©2022 National Council of State Boards of Nursing, Inc. All rights reserved. 3 It has to be part of that education and national certification. Procedural training by itself doesn't qualify competency. It creates misalignment when someone thinks that that's going to keep them in their role.

You can consider a case study scenario as a capsule. And that the selection of this capsule is adaptive. When you open a capsule, the six items are in a linear order for you to take. This is a graphical representation of the adaptive algorithms. The Y-axis is the scale used for both item difficulty and the candidate ability. On this scale, a larger number always indicates a more difficult question and a higher ability. A smaller number always indicates an easier question and a lower ability. The X-axis is simply a tracker of the number of operational questions.

In the middle is the passing standard that separates those individuals at or above the minimal level of competence for safe and effective practice. This is an area where we want to have the greatest precision in order to make sure we are making good decisions. The standard error of measurement is on the Y-axis. This metric is really important because that's the indicator of precision that we use while we're doing the testing. And so for this metric, the lower the value and the better we measure, the less uncertainty we have about a candidate's individual ability with respect to the nursing content on the exam.

If the language demands of the items were govern- ing the difficulty of the items then one would expect to see more deviation from the identity line. If the ESL popula- tion did not understand what was being asked at all, then the probability of a correct response would be approxi- mately 0.25 for all items and the resulting calibrations would pile up around the same point on the Y-axis (exam- ple Figure 3). In this case, this clearly did not happen. Figure 3. What a Comparison of Item Calibrations Would Look Like if the ESL Group Spoke No English. Discussion & Conclusions As expected, the sum of these analyses indicates that there is a relationship between English proficiency and examina- tion performance.

I know we talked about a Leena before. This is not that Leena. And this is a patient of mine. Her name is changed. And I just want to introduce you to her as a little bit of a case study that I hope will carry through this presentation and maybe also into tomorrow. Leena is 26 years old. And is there anything that pops off the screen in your mind when you just look at that very brief description of her from a regulation perspective? Anything freak you out there? Thank you. What... [inaudible] Yeah. What is a 26-year-old doing as a DON?
