There is no clinical judgment model. There is no clinical judgment item. There are, actually, but when we actually developed the item previously, we do not emphasize clinical judgment model, but we have our item actually measure clinical judgment. So, we take looking at one, what current, actually, item measuring clinical judgment and measuring the knowledge component?

But other examples could be in different clinic settings. And that's where that layer four piece comes in, which is okay, now I know I'm in the outpatient clinic or I'm in a community care or a long term care setting, or long term care facility. I have different resources there and I can do different things there than I can if I'm in either an ICU or an emergency department or a PACU. So all of these things need to be taken into consideration when looking at the scenario, building the scenario, and then making sound clinical judgments, knowing what your resources are, knowing exactly what's going on with your client big picture.

And they found that most schools used a traditional clinical education model, with students being assigned a patient during clinical experiences and doing whatever care the patient required during that clinical day. We know that this approach leaves considerable learning to chance. In another part of the study, researchers actually went to clinical with students and their faculty and observed what happened. They found that when students use the traditional model, there is significant individual student downtime, faculty time is spent supervising for student and patient safety, and more concerning, that students did not notice significant changes in patient status, or had misunderstandings of what was happening with their patient.

If an item shows DIF, it could mean three things. First, although the statistics were positive for DIF, there is no evidence from reading the item itself that DIF exists. When the DIF panel reviews an item identified by statistical methods as having DIF, the panel thinks there is nothing wrong with it after reviewing it. This is pretty common. Second, there is a genuine group difference, but nursing content requires the concept to be tested.

And, you know, it was clear that even in full practice authority states barriers continue to exist. The waivers that changed majority of APRN practice, in fact, in our state of Tennessee, the waiver was lifted in May. So it was just several short months of being in effect. I know that there are states where the barriers were lifted and remain lifted, and that's positive. In other states, they really will not be reimposed and that's also beneficial as well. But I think the pandemic highlighted that APRNs rose to the occasion to be able to meet healthcare needs and did so without any undue impact when waivers were released.
