To fully embrace this objective, the NCBON needed to reflect on its current approach to imposing discipline sanctions and make necessary process revisions that protect the citizens of North Carolina, and authorized board staff to investigate the possible use of sanctioning guidelines as an option to improve disciplinary processes. A Brief Review of the Literature There have been few studies examining disciplinary actions by BONs and there is little research involving the development and use of sanctioning guidelines as part of the discipline process for BONs. There is, however, information available highlighting the pervasive culture of blame within the health care industry when errors occur.

Epidural or spinal anesthesia should be offered, when appropriate, for management of pain in labor or for delivery. Opioid agonist–antagonist drugs such as butorphanol, nalbuphine, and pentazocine should be avoided because they can precipitate acute withdrawal in patients taking an opioid agonist. Some patients who are physiologically dependent on opioids may not disclose their substance use and health care providers may, there- fore, not be aware of their opioid use.
