Opioid agonist–antagonist drugs such as butorphanol, nalbuphine, and pentazocine should be avoided because they can precipitate acute withdrawal in patients taking an opioid agonist. Some patients who are physiologically dependent on opioids may not disclose their substance use and health care providers may, there- fore, not be aware of their opioid use.
