When a decision is made to initiate opioid therapy, it should be presented to the patient as a therapeutic trial or test for a defined period of time (usually no more than 30 days) and with specified evaluation points including improvement in pain and function. 10 The clinician should explain that progress will be carefully monitored for both benefit and harm in terms of the effects of opioids on the patient’s level of pain, function, and quality of life, as well as to identify any adverse events or risks to safety33.
